Case Manager - Arab French Home Health Care

ARAB FRENCH HOME HEALTH CARE works closely with the patient, family, physician and multidisciplinary team to determine post-acute needs and identify potential barriers. Essential to this role are the core values of patient advocacy, holistic assessment and care planning. Combine the skills of nurses, social workers and insurance reviewers (utilization review) to assess patients' physical, emotional, social and economic situations.
- Serves as a liaison between the patient, physician and payer along the continuum of care
- Determines the level of care appropriate for patient upon discharge
- Works with physicians and nurses to correct utilization deficiencies and delays






- Provides patient care information as requested to contracted insurers and/or reimbursement entities within the guidelines of the confidentiality information policy
- Determines the level of care upon request for admission utilizing evidence-based criteria
- Accompany the Nursing Supervisor and clinicians to patient’s place of residence for assessments.
- Maintain confidentiality of patient’s record.
- Coordinate with the Nursing Supervisor and other healthcare facilities about admission or discharge of the patient.
- Inform the Nursing Supervisor about new referrals or patient’s related complaints

**Education**:
Bachelor’s degree in Nursing is preferred.

**Work Experience**:

- Minimum of two
- (2) years of experience in case management home care/medical center.
- Evidence of progressive leadership experience is preferred.

**Specific Skills**:

- Knowledge of the requirements of government and other regulatory bodies
- Ability to deal effectively with people at all levels of the health care hierarchy






- Strong oral and written communication skills
- Excellent interpersonal skills
- Strong organizational and time management skills
- Proficiency with Microsoft Office suite
- Fluency in written and spoken English
- Experience with electronic medical record is strongly preferred
- Ability to speak and write in Arabic is preferred

Ability to commute/relocate:

- Al-Ayn: Reliably commute or planning to relocate before starting work (required)
