**Assessment and Planning**: Conduct comprehensive assessments of patients' health status, including physical, emotional, and social factors. Develop individualized care plans based on assessment findings and in collaboration with other healthcare professionals.
- **Medication Management**: Administer medications, injections, and intravenous therapies as prescribed by physicians. Monitor patients for adverse reactions and medication interactions. Educate patients and their families about proper medication administration and potential side effects.
- **Wound Care**: Perform wound care, including cleaning, dressing changes, and assessment of wound healing. Monitor wounds for signs of infection or other complications.
Provide patient and family education on wound care techniques and prevention strategies.
- **Chronic Disease Management**: Assist patients with chronic conditions such as diabetes, heart disease, or respiratory disorders in managing their symptoms and adhering to treatment plans. Educate patients about lifestyle modifications, medication management, and self-care techniques to optimize health outcomes.
- **Monitoring and Evaluation**: Monitor patients' vital signs, symptoms, and overall health status during home visits. Evaluate the effectiveness of interventions and make adjustments to the care plan as needed. Communicate any changes in the patient's condition to the healthcare team and coordinate follow-up care as necessary.
- **Collaboration and Communication**: Collaborate with physicians, nurse practitioners, therapists, and other healthcare professionals to coordinate patient care and ensure continuity of services. Communicate regularly with the healthcare team to provide updates on patient progress,
changes in condition, and any concerns or challenges encountered during home visits.
- **Documentation**: Maintain accurate and up-to-date documentation of all patient assessments, care plans, interventions, and communications in accordance with regulatory standards and agency policies. Ensure that documentation is thorough, complete, and reflects the quality of care provided to each patient.
- **Safety and Infection Control**: Adhere to safety protocols and infection control measures to prevent the spread of infections and ensure a safe environment for patients and caregivers.
Follow established procedures for handling medical equipment, disposing of hazardous materials, and maintaining cleanliness in the homecare setting.
**Salary**: AED8,500.00 - AED9,000.00 per month
